I hope you've thought about your strengths, weaknesses, achievements over the last year along with your objectives for the next year. And where do you want to be in 5 years time?

I used to love appraisals, perfect time to ask for a payrise. Print of some market figures and bingo, another 3.25p an hour

My former employer used to insist that an appraisal was not the forum for discussion of salary matters. Discussion of salary matters was for "pay reviews", which always seemed to get delayed for years at a time.

Ah yes, I remember now, 360 appraisals.

You get to evaluate your boss first and then he evaluates your performance.

9 a) How do you feel you are being managed?

I sense a lack of direction in mid to upper management. This is beginning to make me feel uncomfortable. I feel frustrated!!!

                You should write,

Listening to my managers voice makes my teeth grind rather harshly, then I suddenly get the urge to cut myself with a paper clip.

Sorry, I know this is a little off-topic, and a bit self-indulgent, but I just want to get it off my chest:

My last permi-boss was an UTTER TOTAL

                  Stupid, jumped up, self-important

                  God I'd love to
                  right up his

                  ...kin er!!!!!!
