    Client satisfaction form

    Hi,

I'm considering drawing up a client satisfaction form - something simple with a few graded questions (very poor -> very good, or strongly disagree -> strongly agree), plus space for good and bad comments.*

My thinking is that on the one hand it would be good to hear about any even minor dissatisfiers early on, to allow time to improve things before renewal comes up; and on the other hand to gather positive scores and comments for reference purposes in seeking future clients.

It's not IR35 motivated, but obviously I would want its wording to support an outside status.*

Does anyone have any thoughts, advice or experience around this? *

Can't you just take him for an informal coffee/review session at the end of your gig rather than giving him more paperwork? If you listen you should be able to pick up all the information you need and can save the stuff you think is useful.

I find feedback forms a total waste of time to be honest. People tend to tick somewhere above good randomly just to finish the thing off and get on with their normal work.
