    Who worked on this?

    Another public sector IT system going tits up. Surely not...

    For completeness, I never worked on this.

          These would be covered by some pretty basic test cases.

            If the errors are mostly in the state's favour, I wouldn't put it past the last Government to have had the records tweaked deliberately to up the estimated tax take so they could release over-optimistic estimates of revenues before the election and thus make more convincing their claims that a recovery was underway.

              How did this happen? Who's to blame? Well certainly there are those more responsible than others, and they will be held accountable, but again truth be told, if you're looking for the guilty, you need only look into a mirror.
