      How did this happen? Who's to blame? Well certainly there are those more responsible than others, and they will be held accountable, but again truth be told, if you're looking for the guilty, you need only look into a mirror.

          I thought that at first, but the explosion seems to be off to the left. I guess it's possible the bottom of his barrel exploded but unfortunately the cameraman didn't stick around long enough for us to see the aftermath...
