How did this happen? Who's to blame? Well certainly there are those more responsible than others, and they will be held accountable, but again truth be told, if you're looking for the guilty, you need only look into a mirror.

I've had some dodgy landings in a 767 n BA's flights into some of the smaller airports in the Caribbean during hurricane season, but nothing has been as bad as that.How did this happen?
