Recently we have missed many SLAs due to slow database build times (outside my remit).

The PM keeps walking out of emergency meetings saying "He's heard enough of all this bulltulip and nonsense <bleep> , <bleep>....." and storming off in a mood despite the team's best efforts to resolve on-going issues.

Any advice would be welcome as I wish to speak to him later regarding a contract extention and a rate increase.

Ask him why, if it's so simple, doesn't he stop acting like a petulant child and fix it himself?

When he twats you, get him fired and take his job. Then give yourself a rise.

you have written him a completely factual report with the causes and log files to back it up. No speculation or conclusions required, you can add positive recommendations to avoid in future (show working) .

Assuming at least one senior manager has some technical knowledge that should deflect the pain.

Stop inviting him to meetings and get the team to take the decisions without him. Sooner or later he'll get angry and ask why he wasn't in on a decision, then you tell him he gave the impression of not wanting to take part in the meetings where decisions are made.
