Yes it can, you can do Furlough with annual salary, its mostly irrelevant to HMRC, all they check is that you had PAYE RTI submission before March 2020, you will also need to use the CJRS calculator...
Yes that makes sense, I do my own accounts and could if I wanted pay nil months, pay double in one or as suggested pay 11 at nil then one annual amount at the end but its still a monthly scheme.

Yes..You can apply for minimum of 3 weeks but you cant apply more than 2 weeks in advance. i.e if you are furloughed today, you have to wait a week to apply (assuming you are applying for 3 weeks...

What are you using to do the calculations ? Xero, Freeagent and Brightpay have all introduced new software to calculate furlough, Xero & Brightpay did some free online presentations that I found...
You can do this online too..if you go to your tax code hmrc page it lists the dividends it expects you to get based on year before, obviously if you arnt going to get any it messes up your code.
