Adults Social Worker | Preparing for Adulthood Team (Transitions) | Richmond & Wandsworth Council
Rate: Up to £32 per hour (umbrella) | Hybrid working | 36 hours per week

Caritas Recruitment are currently recruiting for an experienced Adults Social Worker to join Wandsworth Council's Preparing for Adulthood (Transitions) Team.

This is a fantastic opportunity for a qualified professional passionate about supporting young people as they transition into adulthood, helping them achieve independence and positive outcomes. Key Responsibilities

Manage a caseload of approximately 20 cases, plus participation in the Duty Rota one week at a time.

Develop and implement transition plans for young adults, supporting their move from children's to adult services.

Undertake assessments, risk management, and outcome-based planning in line with statutory guidance.

Work collaboratively with children's services, education providers, CAMHS, and adult mental health teams to ensure a joined-up approach.

Empower young people and their families to make informed decisions, promoting independence and inclusion.

Maintain accurate case recording, contribute to multi-agency meetings, and ensure safeguarding responsibilities are met.

What We're Looking For

Qualified Social Worker with SWE registration

Minimum of 2 years' post-qualified experience in adult or transitions social care

Strong communication, empathy, and advocacy skills

Experience in transitions planning and partnership working across education, health, and social care

Full UK driving licence and access to a vehicle preferred

Right to Work in the UK (please note sponsorship is not available)

Role Details

Rate: Up to £32 per hour (umbrella)

Hours: 36 hours per week, Monday-Friday, 9am-5pm

Duration: Initial 12-week assignment

Hybrid Working: Combination of home working, in-person assessments, and 2 office days per week

Location: Wandsworth Council

Equipment: Laptop provided (collection required)
