    Personal Services Company Compliance Questionnaire

I am reluctantly having to use Aston Carter as the agent for my next contract. They have sent me a PSC form which I want to make sure that I am happy completing correctly.

There was a similar thread a few years ago, but that was when there was an existing contract in place.
    1. I notice now that the they are combining/mixing up the terms PSC and OLC, is this of any relevance?
    2. Also does answering "Yes" to question 4 affect my perceived right of substitution?

The form has the following relevant section:

    Section B: Personal Service Companies
    1. Are you the Sole Director of the OLC?
    2. Do you own 100% of the shares in the OLC?
    3. If no, what is your percentage and who are the other major shareholders?
    4. Are you the sole worker providing services through the OLC?
    5. If no, please specify details below
    6. Please confirm that all payments made by Aston Carter are paid directly into the OLC’s bank account (details given above in section A)
    7. Please confirm that the OLC’s bank account is based in the United Kingdom?
    8. Do you sign contracts for and on behalf of the OLC?
    9. Does the OLC hold professional indemnity insurance?
    10. If yes, please confirm the amount the OLC holds and that the OLC is the insured and that such insurance is not part of a block policy held by a third party:
    11. Please provide the details of any accountancy or legal advisors of the
    Company Name:
    Address:

Same as a PLC - something that is undefined legally and has no meaning. Although I'm guessing is a carp acronym for One Man Llimited company. Who knows or cares?

But 'cos I'm bored, to answer the OP's question, 95% of that information is available from Companies House for a fiver. The bits that aren't - mainly the bank account details, but I rather hope they know where they are sending the money so why do they need to ask anyway - are not their concern.
      Blog? What blog...?

Apart from question 9, I can't see that the answers to any of these questions are any of their business TBH. Or am I being naive here?

Well I haven't provided them yet, it's the first time my company has worked with them. I wouldn't mind getting paid
