Where can I find out how many shares were approved (but not allocated) as part of my company formation? I can't find the information on the documentation I have. Would it be in the information held by Companies House if I pay for a report from them?

As Waldorf has stated, the number of shares authorised and the original number of shares subscribed to should be in the Memorandum and the Articles of Association.
You should have copies of these but if not they should be available to download from Companies House (for a fee) - assuming a private limited company incorporated in England and Wales.
I wont ask how you have been sure that you have been making the correct dividend payments ....Comment

Thanks for your replies. The details are not in the memorandum. I've been doing some reading and it appears that because my company was formed after 1/10/09 things are done differently and I don't have any shares waiting to be allocated. I just need to work out how to get new ones - I'm in the middle of reading the details...Loopy LooComment

Under the Companies Act 2006 limited companies created from 1st October 2009 onwards have no concept of "authorised share capital". The shares issued at the time the company is formed are listed in the company formation documents and further shares can be issued according to the company's rules and Companies House notified accordingly.
